Refund Process:

  1. Complete the Refund Request Form   DOWNLOAD FORM
  1. To obtain your refund, mail the completed Refund Request Form along with the original sales receipt and the original UPC label from the product to:

  1. Upon receipt of the UPC label and sales receipt, we will refund you the purchase price minus any coupon(s) but including sales tax based upon the information provided on the receipt.
